Patient Portal Instructions
We’re looking forward to seeing you soon! For contactless service, we need the forms in your patient portal completed prior to your appointment.
We’ll send an email with instructions for online forms through the patient portal. The email will include a link to the patient portal, which you can access here, and an attachment with your username and password for first-time users. The password provided is temporary; change it when you log in.
We’ll text your family when new forms are added to your patient portal so you can fill them out prior to your appointment. Use your existing login information.
If you need assistance logging in, updating your email, or resetting your password, just contact us by phone or text.
To access new forms in the portal, click on the patient’s name located in the drop-down menu. If you have more than one child, you’ll be able to change names under the same tab to fill out forms for each.
